ABOUT CONDITIONS REPORT

This report is not an invoice.

THIS REPORT IS INTENDED TO PROVIDE THE CUSTOMER WITH A DESCRIPTION OF WHAT THE CONDITIONS WERE WHEN THE TECHNICIAN ARRIVED, THE WORK THEY PERFORMED, AND THE CONDITION OF THE DOOR WHEN THE TECHNICIAN COMPLETED THE WORK APPROVED BY THE CUSTOMER.  A DETAILED RECEIPT FOR THE WORK PERFORMED WILL COME IN A SEPARATE EMAIL.

THE TERMS FOUND IN THE CONDITION REPORT ARE DEFINED AS FOLLOWS: 

Not at Risk – This part is currently working. There is no way to service this part in order to prevent problems, and there is no chance that this part can cause future problems to the door or opener system.

Maintenance Required – This part is working and in good order; however, this part requires maintenance, lubrication, or yearly professional attention that is NOT covered under warranty. Garage doors must be maintained regularly. Regular maintenance is not a warrantable concern.

At Risk – This part is at risk due to missing or failing parts that are marked  bad. This is expected to have the potential for future problems and will NOT be covered by warranty.

Bad – This part has failed or is failing and needs to be changed out immediately in order for the door or opener to function properly. Safety is often considered an issue. Some parts are  bad due to age and life expectancy. These parts are not covered by warranty, and may cause damage to other components on the door and opener systems.

Safety eyes and sensitivity adjustments do NOT require a professional and are never covered by warranty – $119.00 to adjust. Previous customers will only be charged $49.00 if payment is made at time of adjustment. Frequency Interference is not a warrantable concern. (One Clear Choice will work closely with customers affected in order to find a “cost only” resolution to frequency interference problems.)

Haul Off and Disposal – Haul off and disposal service is NOT included on any discounted purchase with regard to openers and door installs.

Old Parts – These parts will be considered as trash and disposed of. Parts will be left with customer by request only at time of job completion.

Verbal Agreements – Any verbal agreements made between customer and technician must be in writing and approved by the office in order to be considered valid.

Invoice – For all garage door orders, customer agrees any changes will be requested in writing. Any change after order is processed, a minimum of $100.00 and any special order deposit will be charged.

Low Price Guarantee – 100% of amount paid on Measure and Quotes ($25.00) or Service and Repair, quotes only, ($69.00) will be refunded if One Clear Choice is unable to beat any written quote produced by an accredited BBB company, generated on company letterhead while present at the job site. (No faxes, business cards, emails, verbal agreements, or generic invoices will be considered. Service and repair quotes with parts installed will NOT be eligible for low price guarantee. Completed jobs or any portion that has been completed will NOT be eligible for low price guarantee.)

CONDITIONAL DISCOUNT DISCLOSURE AND CUSTOMER AGREEMENT

One Clear Choice takes pride in going above and beyond with regard to customer service issues. We are thankful for the opportunity to serve you. If for some reason you are not completely satisfied, please let us know. Due to the extensive amount of time it takes to resolve situations involving the circumstances below, and in an effort to provide solutions that are in the best interest of all parties involved, One Clear Choice requires your cooperation and understanding. All discounts are conditional and will be forfeited, billed, and payable upon receipt. Customer understands and agrees to vacate any discount provided to the customer through coupon, office agreement, tech discount, screaming deal, etc. if the conditions listed below exist.

1. Insufficient funds, closed accounts, cancelled checks and declined credit cards will result in the loss of all discounts. A minimum $35.00 charge will be added to the bill for bad checks. Other charges may apply.

2. All invoices are due upon completion of work. Refusal to pay any completed portion of a job will result in the loss of all discounts.
